An employee was welding a metal strap on both sides of a 20-foot long pipe. There was a welding machine on each side of the pipe. The pipes were placed on a conveyor which moved the pipes toward the pre-stress wiring machine. While the employee was walking between the pipes, a pipe rolled backward off the conveyor and caught the employee between the pipes. The employee sustained broken ribs and was hospitalized. | |||||

An employee was instructing another employee on how to conduct a moisture test by placing baked cookies coming out of the oven (on a conveyor belt) on top of the fan's guard enclosure. A series of three to four fans are attached to the belt to blow down on the cookies (cooling) as they come out of the oven. These fans could be located 6 to 12 inches above the conveyor belt.
